Download and print the application form (Appendix ‘A’) and admit card format (Appendix ‘B’/’C’) on A4 size paper.
Fill in all required fields accurately in English using BLOCK letters.
Attach self-attested copies of educational certificates, ITI certificates, experience documents, caste/category certificates, and ID proof.
Affix 3 recent passport-size photographs (1 on the application form and 2 on duplicate admit cards).
Enclose a self-addressed envelope (30 cm x 12 cm) affixed with a postal stamp of Rs. 55/-.
